Your completed registration form MUST include registration, signature page, and post dated cheques before your child is considered registered for the 2010-2011 season. TMHA is under no obligation to release a player from its organization because the player does not meet the registration deadline(s).

ROOKIE PROGRAM: If this is the first time this player is in a Minor Hockey program, then TMHA waives registration fees only, for Tom Thumb and Novice level.  You’re still responsible for the $32 Insurance premium, concession duty and all fundraisers.



REGISTRATION FEE:  Payment in full by Sept. 2, 2010 or 50% due Sept. 2/10 and a PDC dated Dec 31/10 for the final balance. 


No registration fee will be refunded after December 15, 2010. In the event any player is cut from a competitive team, and there is no other place for the player in the hockey program, the registration fee will be refunded.

This Consent Form must be signed by a Member’s (Player’s) Parent or Legal Guardian to authorize the use of his/ her photo and/or name on the TMHA's website during the 2010/11 hockey season.

CONCESSION DUTY OPTIONS:


A:  I wish to payout my concession duty:  Cheque dated October 15/10, for $200.00 for each registered player.

B:  I will work my concession duty: PDC dated March 1/11, for $200.00 for each registered player.
 
  This cheque will be cashed if you do not work your designated shifts. If you cannot work a shift, switching with another parent is acceptable. However, if we are required to hire it out, you will be required to pay $50.00 per shift, prior to the shift schedule. 48 hours notice must be given in the event you are unable to work, or a fine may be levied as determined by TMH executive.

Persons from the ages of 12-14 years may assist in the concession ONLY WHEN ACCOMPANIED BY THEIR PARENT/GUARDIAN AND ARE RESTRICTED TO THE SERVING OF ‘COLD PRODUCT ONLY’. Persons under the age of 12 are strictly prohibited from entering the concession area.

FUNDRAISING  OPTIONS


  The fundraising activities for the 2010/2011 season will be tickets to WEM Waterpark for Friday, January 7, 2011; and chocolate almonds – 3 cases per registered player to a maximum 6 cases per family. 


A) I agree to participate in the TMHA FUNDRAISER. I will provide a $250.00 PDC for Dec 15, 2010 and a $250.00 PDC for March 1, 2011 for each registered player. I understand that these cheques will be cashed if I do not meet my fundraising obligation. I understand that I am responsible for 100% of the fundraising product and its expected revenue and that I cannot return unsold product. No refunds will be given.

BI do not wish to participate in the TMHA Fundraiser. I will provide an additional $250.00 cheque dated Sept 02, 2010, with my registration fee payment or first installment, for each registered participant.

* NB* Regardless of the fundraising option chosen above, each team will be required to do a TEAM fundraiser for the year, which includes but is not restricted to the teams home tournament. 


The AGM is set for 7:00 p.m. on Tuesday, April 19, 2011. This is where TMHA members have a voice and a vote. TMHA cannot function without the support of its members.  It is expected that a representative from each family will attend. A $50 bond per family in the form of a PDC dated April 20, 2011 is required with your registration. Your cheque will be returned to you when you attend the AGM once the meeting has been adjourned, or it will be cashed should you not attend.
